Kind, supportive coaching honors the dignity of each individual, encouraging continual growth in independence with the safety net of help available when needed through our Supported Living program.

Building Relationships

Our Supported Living program helps prevent isolation and fosters friendship building. We encourage becoming active in the local community through hobbies, volunteering, and activities. We partner with other services to form a network of personalized support. We help people explore and develop their own interests to become engaged, meaningful members of their community. These supports reduce loneliness by offering opportunities to build relationships and meaningful connections.

Supporting Your Independence

Your support team includes Direct Support Professionals, Program Managers, and Leadership from our Wall Community Options staff, all dedicated to your goals for independence. Our team members are carefully selected for expertise, experience, warmth, and compassion. The leadership team for Supported Living has over 45 years of combined experience in person-centered support and independent living for persons with disabilities.

A Team Approach

We reach beyond our own organization to work with support coordinators, landlords, job coaches, medical professionals, social security income (SSI) administrators, SRAP (State Rental Assistance Program) housing authorities, and family members. We help manage the details of life to keep them from becoming overwhelming: bills, budgeting, medical coordination, nutrition, housekeeping, and medication management, and reliable transportation. Together, these supports create a strong foundation for independent living for persons with disabilities.

RN Care Management Support

Medical support by Registered Nurses increases quality of care.

Wall Residences Supported Living services are supported by a team of RN Care Managers who ensure best practice standards for medical support are upheld. All Wall medical trainings are led by Registered Nurses. 

Our RN Care Management

  • Supports program management understanding of health management
  • Acts as a resource for Providers managing individual’s health needs
  • Ensures standards of care through protocol development and RN training
  • Develops medical trainings based on current best practices
  • Reviews all new admissions for medical needs for proactive health management
  • Advocates for individuals with medical providers as needed, as well as supporting and educating program managers and Providers on how to be medical advocates.
  • Develops policies and processes to minimize preventable accidents and illnesses
  • Provides a high level of specialized medical knowledge

My Home with Individual Supports

Individuals are able to remain in their own home or apartment with daily check-ins and professional on-call support available 24/7.

Individual Supportive Housing

Individuals choose an apartment or home subleased from the housing provider using a rent subsidy. Daily checks and professional on-call support are available 24/7.

Clustered Supportive Housing

Supported individuals occupy several units in the same property, and independent living support services layer on top to provide security and stability with activities of daily living. Professional staff is available 24/7.

No. Medical needs and post-surgical support are not deterrents unless the condition is chronic and prevents staying home alone long-term.

Yes! Many individuals combine Supported Living with Community Engagement, Day Support, Therapeutic Consultation, or other services. Our team can coordinate with colleagues from other Wall programs to ensure seamless support.

No. This service is designed for adults living independently or with friends.
