Admissions Process
Our signature matching process focuses on forming long-term relationships based on mutual selection.
Admissions staff meet with you and your family to learn about your needs and desires, then identify potential matches. Visits let you meet the Provider and experience the service. Once a match is found, a planning meeting is scheduled and all medical documentation must be submitted. A start date is set after the service plan is developed.

Choice of Location and Provider
Wall Residences has many available service options ready to go across the state at any time and can develop services specific to most any person, given enough time. We have the most professional, experienced, and well-trained Providers in the state. Our Providers are dedicated to the individuals they serve and offering the best care possible.
Careful Supervision and Support
Families have peace of mind, knowing every service is reviewed regularly by qualified Program Management staff. Unannounced visits occur at least twice per year with announced visits monthly. Wall Residences staff and providers work as a team with the individual’s family, case manager/support coordinator, and other service providers to ensure the success of the individual in the service. The Quality Assurance Team and Regional Coordinators are available at any time, day or night, to help Providers with critical incidents.
Relationships are Key
Family Members are encouraged to maintain relationships with the person who receives support. Visits are encouraged. When that is not possible, pictures, cards, texts, phone calls, and video chats are used to facilitate relationships from a distance. Relationships between the individual’s family and the Provider family are common and the frequent outcome of our careful matching and long-term placements.
Transparent, Thorough Communication
Wall Residences documentation and support is complete, accurate, and timely.
Detailed quarterly reports to Support Coordinators and authorized representative or legal guardians include financial information, medical appointments, and outcomes. If critical incidents arise, the Quality Assurance Team and Regional Coordinators are available to help Providers problem solve at any time, day or night. Of course, in a medical emergency, all staff are instructed to contact local emergency services first.

Documentation
Wall Residences will need to collect documentation for new referrals from the support coordinator - case manager assigned to the individual.
Documentation needed may include:
- Psychological Evaluation
- Current Physical/List of Medications
- Supports Intensity Scale (SIS)
- Virginia Individual Developmental Disability Eligibility Survey (VIDES)
- Person Centered Plan
- IEP (children only)
- Behavioral Support Plan
- REACH Crisis Education Prevention Plan
- Sociocultural Assessment (Children only)

We accept the Community Living (CL) waiver and Children’s Services Act (CSA) funding.
Our team uses a multitude of factors to pair individuals with the right service for their needs. Items considered include:
- Location desired
- Rural, urban, or suburban setting
- Skillset of the Provider vs the support needs of the individual
- Personality
- Likes/Dislikes
- Mobility/accessibility needs
- Animals in the home or no animals in the home
- Other preferences expressed by the individual
Documentation needed may include Psychological Evaluation, Current Physical/List of Medications, Supports Intensity Scale (SIS), Virginia Individual Developmental Disability Eligibility Survey (VIDES), Person Centered Plan, IEP (children only), Behavioral Support Plan, REACH Crisis Education Prevention Plan, and/or Sociocultural Assessment (Children only).
Medication orders that include the prescriber’s signature are needed once a placement is identified before move-in can occur. Guardianship documents (if applicable) are also needed once a placement is secured.
