The Basics
The support you need as you live independently or with family.
This service provides scheduled hours weekly of one-on-one support by a Direct Support Professional.
Payment Accepted
FIS (Family & Individual Supports) Waiver or CL (Community Living) Waiver. Private pay available.
Hours of Support
Ten or more hours of scheduled one-on-one support per week. Hours are based on the Plan for Supports.
Staffing
Direct Support Professional, supported by Program Management and RN Care Management.

Support in Your Space
This program supports individuals living in their own home, either by themselves, with their natural family, with a partner, or with a friend(s). Strengths and needs are assessed to determine how you can best be supported to live as independently as possible, with maximum quality of life in your home and community. This is designed as a one-on-one service with a maximum of 60 hours of weekly support.Â
What Support is Available?
Your Direct Support Professional can help with personal care, medication management, community involvement, and independent living skills. In Home support can occur at home or in the community. Assistance may
include housekeeping, cooking, grocery shopping, budgeting skills, transportation to medical appointments, and exploring the community for shopping, volunteering, or leisure activities.

Wall Residences is an employee-owned agency, leading to longer tenure for Direct Support Professionals than is typical in our industry. This contributes to more stability and expertise among the DSPs who serve you in an In Home environment. As in our residential services, we aim to carefully match you with a DSP for long-term relationship building.Â

RN Care Management Support
Medical support by Registered Nurses increases quality of care.
Wall Residences In Home services are supported by a team of RN Care Managers, who ensure best-practice standards for medical support are upheld. All Wall medical trainings are led by Registered Nurses.Â
Our RN Care Management
- Supports program management understanding of health management
- Acts as a resource for Providers managing individual’s health needs
- Ensures standards of care through protocol development and RN training
- Develops medical trainings based on current best practices
- Reviews all new admissions for medical needs for proactive health management
- Advocates for individuals with medical providers as needed, as well as supporting and educating program managers and Providers on how to be medical advocates.
- Develops policies and processes to minimize preventable accidents and illnesses
- Provides a high level of specialized medical knowledge

Yes. In Home is compatible with consumer-directed services (Moms in Motion, etc.), Community Engagement, Community Coaching, Group Day, and personal response units (call bell, tablet, etc.).Â
No, In Home provides scheduled support based on a set number of hours per week (minimum 10) with limitations to flexibility. Supports can be pre-planned for changes in schedule but staff call-outs are considered cancellation for the day. Emergency coverage is not provided. For on-call support, consider Supported Living services. For round-the-clock support, consider Sponsored Residential or Group Home services.
In Home services are billed hourly and only for activities when the supported individual is present. Hours are limited to those outlined in the Plan for Supports and cannot increase without a state-approved addendum.
No. Family cannot provide In Home support, but In Home can be used alongside Consumer Directed hours, Community Engagement, Community Coaching, Group Day, and personal response units.
